The Butcher's Cleaver is an unusual Diablo 4 weapon for players who want their damage to improve during the final part of an enemy's health bar. Unlike a weapon that simply raises damage at all times, this Ancestral Unique Axe is built around the Injured condition. That makes the key question practical rather than theoretical: does your build reach that health threshold quickly enough to make the weapon worthwhile? If you are comparing Diablo IV Items, the Cleaver deserves attention for its reliable finishing pressure and its useful effect after an Elite kill, but it is not automatically the best choice for every encounter.

What The Butcher's Cleaver Actually Provides

The weapon has 1,085 Damage per Second, 790-1,184 Damage per Hit, and 1.10 Attacks per Second. Its guaranteed base statistic is +25% Critical Strike Damage, giving the axe value even before its Unique Power is considered.

Its main effect causes attacks to deal Critical Strikes against Injured enemies. In practical terms, the weapon is strongest after you have already removed a large portion of the target's health. Critical Chance is not needed to create those guaranteed Critical Strikes during that specific window.

That distinction matters when comparing it with more conventional weapons. The Cleaver does not make every second of a fight equally powerful. It asks your build to handle the opening phase efficiently, then rewards you when the target enters its vulnerable health segment.

How to Build Around the Injured Window

The best use pattern is simple: reduce the enemy normally, identify the Injured threshold, then spend your strongest sustained attacks during the closing phase.

◆ Prioritize reliable damage before the target becomes Injured, because the weapon offers less direct value during that opening portion.

◆ Treat Critical Strike Damage as especially attractive, since the weapon already supplies guaranteed Critical Strikes against qualifying targets.

◆ Use the 1.10 attacks per second to keep pressure steady once the target reaches its final health segment.

◆ Avoid judging the axe only by early-hit numbers, because its value is concentrated near the end of the health bar.

This makes the weapon more appealing for players who dislike slow finishing phases against durable enemies. It is less convincing when enemies die before the Injured condition has time to matter.

The Elite Kill Effect Changes Pack Fights

The Cleaver has a second effect that activates when an Elite is killed. Nearby enemies are Feared and Slowed by 95% for 2 seconds. This is not a permanent defensive layer, but it creates a brief opening after a priority target falls.

◆ Use the first Elite as the target for the weapon's finishing damage.

◆ Take advantage of the two-second control window to reposition or select the next dangerous enemy.

◆ Expect the effect to matter more when several Elites are close together than when one target is isolated.

The result is a repeating rhythm: weaken one priority target, finish it during the guaranteed-Critical phase, then use the temporary crowd control to reduce pressure from the surrounding group. This gives the Cleaver more practical value in dense Elite encounters than its single-target description initially suggests.

Where to Farm It and Who Should Use It

The Butcher's Cleaver is currently listed in The Butcher's Unique pool, with The Butcher placed in The Broiler in Gea Kul. Players specifically pursuing the axe can therefore focus on that associated Lair Boss route rather than relying only on general drops. The process still depends on random results, so a targeted route improves focus without promising a quick acquisition.

The weapon is most suitable for endgame players who regularly fight Elite groups and can push enemies into the Injured state without wasting too much time. It is a weaker fit for builds that already eliminate ordinary targets instantly or rely heavily on a different weapon's always-active benefits.
